SCAN and The Springs Living Partner to Bring a More Connected Healthcare Experience to Older Adults in Oregon

Partnership brings together two organizations with decades of experience serving older adults to more closely connect healthcare with where residents live

LONG BEACH, Calif. and PORTLAND, Ore., Sept. 23, 2026 — SCAN Health Plan (SCAN), one of the nation’s largest not-for-profit Medicare Advantage plans, today announced a new partnership with The Springs Living, a senior living organization serving communities across the Pacific Northwest.

The partnership marks SCAN’s expansion into Oregon’s Clackamas and Washington counties. In addition, SCAN and The Springs Living will launch co-branded products that will be available January 1, 2027, designed to closely connect healthcare with the day-to-day support older adults receive in senior living communities.

Healthcare often becomes more complex as people age, with providers, health plans and senior living communities operating independently. This can leave older adults and their families responsible for navigating disconnected systems and coordinating care themselves. Together, SCAN and The Springs Living are rethinking how healthcare can support older adults by more closely connecting care to where they live.

SCAN brings nearly 50 years of expertise in serving older adults with complex healthcare needs, including through its Institutional Special Needs Plan (I-SNP), which is the largest in California and Arizona. I-SNPs are a type of Medicare Advantage plan designed for individuals who require an institutional level of care and reside in certain institutional orqualifying community settings. The Springs Living understands how to create environments where older adults can thrive and brings insight into the aging healthcare journey.

The organizations are working to connect their capabilities and function as one team around the residents with shared accountability for experiences and outcomes. For residents and their families, this will result in healthcare that is easier to access and navigate, better coordinated among the people involved in the resident’s care, and provides earlier recognition of changing needs.

The initial collaboration will serve The Springs Living communities in the Portland metropolitan area.
