A SCAN Medicare Plan (HMO-POS I-SNP), is designed exclusively for eligible residents of The Springs Living. Created for residents who want a more connected healthcare experience, Springs Advantage combines SCAN's Medicare expertise with The Springs Living's resident-centered approach. Members receive coordinated, community-based care along with comprehensive benefits designed to support their health, independence, and quality of life, helping bring healthcare closer to home.
Springs Together
A SCAN Medicare Plan (HMO-POS I-SNP), is designed exclusively for eligible residents of The Springs Living.Designed for eligible residents who qualify for low-income subsidies and other assistance programs, Springs Together offers the same personalized, coordinated care experience while making high-quality healthcare more accessible. Members benefit from a dedicated care team, valuable supplemental benefits, and a care model focused on early support, prevention, and helping residents remain well in the community they call home.

Residents and families will have opportunities to learn more through upcoming community Town Halls, where they can ask questions and hear more about the program. Check with your community for the date and details of your local Town Hall.
Beginning October 15, eligible residents will also be able to explore plan details and enrollment information online.
