New plans offer onsite primary care, 24/7 clinical support and personalized benefits designed to simplify healthcare for residents and their families
LONG BEACH, CA. and PORTLAND, OR., Oct. 27, 2026 — SCAN Health Plan
(SCAN), one of the nation’s largest not-for-profit Medicare Advantage plans and The
Springs Living, a senior living organization serving communities across the Pacific Northwest, today announced the launch of two co-branded Institutional Special Needs plans (I-SNPs) designed to bring coordinated medical care directly to eligible residents of The Springs Living Communities in Oregon.
Available beginning January 1, 2027, in Oregon’s Washington and Clackamas counties, Springs Advantage, a SCAN Medicare Plan and Springs Together, a SCAN
Medicare Plan combines onsite primary care, around-the-clock clinical support and personalized care coordination with Medicare Advantage benefits tailored to residents' needs. By more closely integrating healthcare services with the day-to-day support residents receive, the plans aim to simplify access to care, improve coordination among providers and help older adults maintain their health and independence in the communities they call home.
“Primary care should mean having a doctor who knows you, meets you where you live and takes the time to understand what matters most to you,” said Dr. Daniel Croymans, Vice President of Value Based Care, SCAN Health Plan. “Together with The Springs Living, we designed these plans around that belief, with a dedicated care team that works alongside community staff who knows them best. By working as one team around each resident, we can move beyond reacting to illness and focus on helping people stay well.”
The new plans build on SCAN’s and The Springs Living's commitment to create a healthcare experience designed specifically around the needs of senior living residents. Rather than requiring residents and families to navigate multiple providers and services independently, the plans offer a coordinated approach that brings healthcare closer to home and helps connect medical providers with the support residents receive in their communities.
Key elements of the care model include:
- Onsite primary care: Convenient access to primary care visits within The Springs Living communities, reducing the need for residents to travel for routine medical appointments.
- 24/7 clinical triage: Around-the-clock access to clinical support to help address health concerns, identify changes in condition and guide residents to appropriate care.
- Coordinated care transitions: Support when residents move between hospitals, rehabilitation facilities and their senior living communities to promote continuity of care and reduce gaps in communication.
- Proactive, personalized care: An emphasis on prevention, early identification of health needs and coordination across healthcare providers to help residents manage chronic conditions and maintain their well-being.
- $0 copays for primary care provider visits.
- $0 copays for Tier 1 and Tier 2 prescription drugs.*
- $55 monthly plan premium.
- Up to $700 annually in FlexEssentials allowances for eligible over-the-counter items. Quarterly limits apply.

“The healthcare system has become increasingly complicated for older adults and their families to navigate. The time is now to create a smoother, more connected experience,” said Brenda Connelly, President, The Springs Living. “By bringing healthcare into our communities, we’re making care more accessible, coordinated, and personalized for residents of The Springs Living. Our partnership, integrated care model, and health plan supports healthier aging, enhances overall well-being, and provides our community teams with the tools and clinical support needed to better serve residents every day.”
In addition to the coordinated care model, both plans offer specialized benefits designed to support residents' overall health and well-being.
Springs Advantage, a SCAN Medicare Plan (HMO-POS I-SNP) offers comprehensive Medicare Advantage coverage with personalized healthcare support and supplemental benefits, including:
Springs Together, a SCAN Medicare Plan (HMO-POS I-SNP) is designed for eligible residents who have both Medicare and Medicaid. While this plan is designed for those with both Medicare and Medicaid, having Medicaid is not a requirement for joining this plan. This plan offers the same coordinated care experience with benefits that help make healthcare more accessible and affordable, including:
By combining SCAN's experience serving older adults with complex healthcare needs and The Springs Living's understanding of its residents, the organizations are creating a more coordinated approach to healthcare that supports prevention, timely intervention and continuity of care. The launch represents the next step in their shared commitment to helping older adults receive high-quality healthcare while remaining healthy, engaged and independent.
Eligible Medicare beneficiaries can enroll in Springs Advantage, a SCAN Medicare
