Through the plan, residents have access to the Embrace care team, a dedicated group of healthcare professionals who get to know you and work closely with the care team at the community to coordinate your care.
Springs Together
A SCAN Medicare Plan (HMO-POS I-SNP) is designed exclusively for eligible residents of The Springs Living who have Medicaid. This plan provides residents access to the Embrace care team. This group of experienced healthcare professionals will collaborate with the community care team to ensure your healthcare needs are met.
